Ideal Job Description
a. Type of Practice
Outpatient psychiatric specialty clinic with a focus on interventional psychiatry, particularly ketamine-assisted treatment.
b. Location
Boise, Idaho
c. Population Served
Adult patients with treatment-resistant depression, anxiety disorders, PTSD, and other mood disorders who have not had adequate response to standard medications.
d. Position Title
Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)
e. General Job Description
In this role, I would provide psychiatric care within a ketamine clinic setting. Responsibilities would include completing comprehensive psychiatric evaluations and determining whether patients are appropriate candidates for ketamine therapy. I would prescribe and manage psychotropic medications as needed and administer ketamine treatments, including IV infusions and intranasal esketamine in accordance with REMS requirements.
The position also involves closely monitoring patients during and after treatment, including tracking vital signs and managing any adverse reactions. Collaboration with other members of the care team is essential, as is providing thorough patient education and obtaining informed consent regarding the risks and benefits of treatment.
f. Benefits Requirements
The position would offer a competitive salary with productivity-based incentives, along with health, dental, and vision insurance. Additional benefits would include a CME allowance with dedicated education time, malpractice coverage with tail insurance, and a retirement plan such as a 401(k). Support for specialized training in ketamine therapy would also be important.
Certification Plan (PMHNP-BC)
I plan to obtain certification through the American Nurses Credentialing Center (ANCC).
The process will involve graduating from an accredited PMHNP program, submitting an application to the ANCC, and providing transcripts along with verification of clinical hours. After approval, I will schedule and take the board exam. Once certified, I will maintain my credential by completing 75 continuing education hours every five years.
Licensure and Prescriptive Authority (Idaho)
Licensure will be completed through the Idaho Board of Nursing.
After passing the certification exam, I will submit my APRN application, including proof of my RN license, transcripts, and national certification. A background check will also be required.
For prescriptive authority, I will apply at the same time as APRN licensure. Since Idaho is a full practice authority state, I will be able to prescribe independently once licensed.
Malpractice Insurance Plan
Even if my employer provides coverage, I plan to carry an individual malpractice policy for additional protection.
The policy should specifically include psychiatric services as well as procedural treatments such as IV ketamine and intranasal administration. A provider like Nurses Service Organization (NSO) would be a likely option.
Continuing Professional Development Record
To stay organized, I plan to track all continuing education activities using a spreadsheet or an online tracking system. For each course, I will document the title, date completed, number of CE hours, and topic area (such as psychopharmacology, ketamine therapy, or safety monitoring).
I will also keep digital copies of all certificates in a secure cloud-based folder so they are easy to access when needed.
National Provider Identifier (NPI) Plan
After obtaining licensure, I will apply for an NPI through the National Plan and Provider Enumeration System. The process is straightforward and typically only takes a few days.
The NPI will be necessary for billing, credentialing, and insurance paneling.
Timeline / Role Development Plan
Overall, my timeline is realistic, but I’ve refined it slightly:
- Certification: Within 1–2 months after graduation
- Certification Review: About 4–6 weeks of focused study, including practice questions and possibly a review course
- Licensure: Approximately 2–3 months after passing boards
- Prescriptive Authority: Obtained at the same time as licensure
- Controlled Substance Registration (DEA and state): Within 3–5 months after licensure
- •NPI Application: Completed immediately after licensure, bringing the total timeline to around 3-4 months
